Costs

Down the road, you probably won't think about the expenses incurred while going through egg donation or gestational surrogacy to build your family. However, at the beginning, it is common for the cost of treatment to be a barrier at worst or a stress at best. Forward Fertility will always be straight forward regarding costs and an effort is always made to keep them as low as possible, so more people have access to care. The agency fees are listed below and a detailed outline of total estimated costs is available on request. Forward Fertility will not charge a second fee to rematch you, if the first match is not successful. There is no application fee to be considered as a potential client.

You do not pay Forward Fertility until the carrier you are going to match with has: completed a psychological evaluation, your IVF clinic has reviewed her prenatal/delivery/postpartum records, AND you've had an in person meeting with the carrier and her partner (if applicable), facilitated by Christie Olsen, NP at the Forward Fertility office.

COSTS FOR GESTATIONAL SURROGACY

The agency fee for gestational surrogacy is $23,000. 

The Self-Match agency fee (bringing your own gestational carrier and Forward Fertility coordinating your match) is $14,000.

This fee is broken up in three installments, based on milestones we reach.

Installment 1 ($13,000):  Made AFTER you have met the carrier and her partner (if applicable) IN PERSON at a facilitated meeting at the Forward Fertility office.

Installment 2 ($5,000): Made AFTER the carrier has been medically screened by your IVF clinic provider AND you've completed the legal agreement with the carrier. 

Installment 3 ($5,000): Made at 20 weeks gestation.

Should the carrier you match with not work out, the agency fee includes a re-match at no additional agency fee cost.

You should plan on the cost of having a baby via surrogacy could cost $100,000 or more. 

One of the biggest ways you can save on costs is by working with an IVF clinic where you're most likely to have success on the first transfer. You can look up the National success rate and IVF specific success rates at www.sart.org or by clicking here.

Agency fee includes services such as, but are not limited to:

  • Presentation of one or more potential gestational carrier applicants
  • Coordination of IVF clinic reviewing the carrier's records BEFORE you meet the carrier
  • Psychological evaluation, performed via ASRM (American Society of Reproductive Medicine) guidelines
  • In-home interview of all potential gestational carrier and partner (if applicable)
  • Review of medical records of gestational carrier by a women’s health nurse practitioner with two decades of experience
  • In-home meeting with parents-to-be to discuss the surrogacy process and preferences (video conference if necessary)
  • Opportunity to speak with former clients or surrogates as references
  • Coordination of legal requirements from before treatment starts to after delivery
  • Unlimited phone and email contact with Forward Fertility staff; including evenings and weekends, if needed
  • In-person visits or informational meetings with Forward Fertility as desired
  • Travel arrangements as needed
  • Injection training or assistance for you or your surrogate
  • Scheduling medical appointments and locating monitoring IVF clinics for you or your surrogate
  • Coordination of escrow account and confirmation of payments
  • Support and anticipatory guidance for you and your surrogate from the very beginning to after the delivery
  • Professional, responsive, attentive care for you and your surrogate

COSTS FOR EGG DONATION

The agency fee for egg donation is $3,000.  Yup. That's not a typo.  It's the same fee we've had since 2013.  This is one way we can make fertility treatments more affordable.  It also allows more of the money you spend to go to the actual egg donor.   As an example, Conceivabilities (2024) charges $9,500 for the egg donor agency fee. 

The fee is due when you sign the Forward Fertility services agreement. There is no repeat agency fee if the first donor you select does not work out.

Egg donors are compensated based on their request - average compensation is between $8,000-12,000.

The TOTAL out of pocket cost for conceiving with an egg donor (legal, IVF, donor compensation, donor medications) can range from $15,000-30,000.  A cost overview can be emailed you by requesting one at info@forwardfertility.com.

Services include, but are not limited to:

  • Full profiles available online of all egg donors in the database
  • In-person interview of each egg donor, conducted by Christie Olsen, NP
  • Review of egg donor’s medical records by a nurse practitioner to corroborate information given in application and interview
  • In-home meeting with parents-to-be to discuss the egg donation and preferences (video conference if necessary)
  • Opportunity to speak with former clients as references
  • Coordination of legal requirements from before treatment starts to after delivery
  • Unlimited phone and email contact with Forward Fertility staff; including evenings and weekends, if needed
  • In-person visits or informational meetings with Forward Fertility as desired
  • Travel arrangements as needed
  • Injection training or assistance for you or your egg donor
  • Scheduling medical appointments and locating monitoring IVF clinics for you or your egg donor
  • Coordination of escrow account and confirmation of payments
  • Arranging insurance coverage for egg donor and managing claims, in the event of a complication
  • Support and anticipatory guidance for you and your egg donor from the very beginning to after the delivery
  • Professional, responsive, attentive care for you and your egg donor

COSTS FOR GESTATIONAL SURROGACY AND EGG DONATION

The agency fee for gestational surrogacy and egg donation coordination via Forward Fertility is $27,000.

Services include, but are not limited to:

  • Presentation of one or more potential gestational surrogate applicants
  • Full profiles viewable online of all egg donors available
  • In-person interview of each egg donor
  • In-home interview of all potential gestational surrogates and partner (if applicable)
  • Review of medical records of gestational surrogate and egg donor by a women’s health nurse practitioner with 16 years of experience
  • In-home meeting with parents-to-be to discuss the process and preferences (video conference if necessary)
  • Opportunity to speak with former clients or surrogates as references
  • Coordination of legal requirements from before treatment starts to after delivery
  • Unlimited phone and email contact with Forward Fertility staff; including evenings and weekends, if needed
  • In-person visits or informational meetings with Forward Fertility as desired
  • Travel arrangements as needed
  • Injection training or assistance for your egg donor or your surrogate
  • Scheduling medical appointments and locating monitoring IVF clinics for you, your egg donor, or your surrogate
  • Coordination of escrow account and confirmation of payments
  • Support and anticipatory guidance for you, your egg donor, and your surrogate from the very beginning to after the delivery
